The Quality Improvement Goal for our Strategic Plan was
developed by the Clinic members in cooperation with the Quality
committee and approved by our Board of Directors in November
2008. The frameworks for these initiatives are being developed
as hallmarks for quality and safety as part of the overarching
organizational level of coordinated clinical and systematic
improvements in service delivery and clinic operations for
quality improvement.
The Oral Health Wellness Initiative – voted as the quality
project to begin our first of three year strategic plan. The
processes for this initiative are being developed as a template
for planning, implementing and continuing to improve the quality
of healthcare of our vulnerable safety net population. The
screening or assessment examination, education, and data capture
tools are being identified to meet the specific needs in each
category which include infants and children, adolescence,
adults, diabetics, cardiac, pregnancy, tobacco and drug users.
Through advocacy, education, engagement and commitment, the
quality committee continues to work together with the clinic
members to ensure implementation of the quality assurance plans
for both current and new projects. The Medical and Dental
Provider Forums each convene on a quarterly basis and provide
continuing education, shared best practices, review of new
quality initiatives, shared operational efficiencies, managed
care updates, and networking amongst each other.
The Quality Department participates and partners with the
California Primary Care Association, CPCA and California Health
Care Foundation, CHCF in QI peer network groups, Data Accuracy
work groups and Steering committee for disease registry as part
of an eleven consortium statewide collaborative focusing on
adult diabetes.

Emergency Planning for Clinic Preparedness along with other
California consortium participating with CPCA and the Orange
County Health Care Agency, OCHCA as the liaison for Community
Health Centers in Orange County. We have participated in local
and statewide drills to develop policies and procedures for
disaster preparedness and have conducted health center readiness
assessment, preparation and planning.
Kaiser Permanente provided an evidence based practice project
which piloted electronic vital sign implementation in the
ambulatory clinical setting.

Kaiser also offered four consortia scholarships to our members
this year to attend the Institute for Healthcare Improvement -
20th annual National Forum on Quality Improvement in Health
Care. The theme, “the future of health care is ours to imagine.”
This academic opportunity offered key note speakers who are the
leading authorities on health care quality and improvement. Four
days of learning labs, mini courses, keynote speakers and
breakouts sessions providing the new generation of ideas on
spreading and sustaining improvement of healthcare during these
transformational times.
